Last week, your student was given a blue flu vaccine consent form that looks like the one attached to this post. They were to bring the form home to a parent or guardian for review. Flu vaccines are free and will be given here on the 25th of this month by our local health department. The forms need to be completed and returned by this Thursday in order for your child to receive the vaccine here at the school. If your child did not bring home a form, we have them available here at the school. Please return the completed form to the school nurse as soon as possible. Thank you!
